NEW UPDATE:

Our amazing caregivers and management teams have worked hard to keep the COVID-19 virus out of our BAKA Communities.

Beaver Dam Assisted Living & Memory Care Residents and Family Members: We are happy to report that we have ZERO active/suspected cases for both residents and staff members in either building. The Beaver Dam campus has reopened to visitation without restrictions. Please remember to check-in at the front desk, so that you can be properly screened for signs and symptoms before proceeding throughout the community. Thank you!

Emerald Bay Retirement Community & Memory Care Residents and Family Members: Unfortunately, we have had six (6) residents and two (2) staff test positive for COVID-19 at Emerald Bay Retirement Community. For this reason, we will be restricting visitation in Emerald Bay Retirement Community until December 10th. Please see our visitation definition below and note that Emerald Bay Retirement Community will be open to Essential Visitors only until further notice. Emerald Bay Memory Care remains open to visitation at this time. If there is an increase in positive cases in our Memory Care, we will limit visitation to Essential Visitors only and notify the families immediately of the visitation change.

Essential Visitors (EVs):

  • May visit any day of the week between 8 a.m. and 8 p.m. (this includes weekends and holidays).
    • There may be extended wait times for a staff member to answer the door and provide screening/badge services if visitors come when there is no receptionist. Please check dates and times receptionists are assigned to front entrances to avoid waiting.
  • Must be screened for symptoms prior to entering the community for each visit
  • Must pick up a laminated visitor’s pass at the front entrance and wear it in a visible place at all times AND return it to the container marked “badges to be sanitized” prior to leaving the community (located at the entrance/exit). Badges taken out of the community will not be replaced and must be returned to the community to continue visiting during periods of quarantine.
  • Must wear a mask at ALL times (including in the resident apartment)
  • May not stop to visit staff, the director or other residents – please call or email the community to discuss concerns or to ask questions.
  • Must do everything possible to protect our residents and staff – wear a mask, wash hands, visit wearing clean clothing and outerwear, stay 6 feet apart, etc.
  • May not visit during meal times (8a-9a), (12p-1p) & (5p-6p)
  • May not relinquish this position to other family members or friends.
  • May bring in supplies, gifts, personal products, etc. for resident use.

The goal is to help residents stay connected and assure families their loved ones are receiving good care and are safe.

Additional changes:

  • Beauticians will be swabbed and tested prior to entering.
  • Church service personnel will be swabbed and tested prior to entering the community and will provide church services to those negative for COVID. Residents attending services must wear masks at all times. No physical communion will be offered at this time.
  • Hospice personnel and home health services are allowed in the community at all times but must be screened, masked and sanitized prior to entering and must be tested by their individual agencies prior to visiting residents in their apartments.

Unfortunately, we have had two residents and one staff test positive for COVID-19 at Emerald Bay on or before November 26th, 2021. For this reason, we will be restricting visitation in Emerald Bay Retirement Community until December 6th. Please see our visitation definition below and note that Emerald Bay Retirement Community will be open to Essential Visitors only until further notice.

The COVID-19 virus is very serious, but so is our resident’s need to see loved ones. Each resident will be asked to designate two “Essential Visitors” who will be allowed into the community even during quarantined periods.

The Essential Visitors will not be allowed into an apartment when the resident is positive for COVID. This is to keep the virus from spreading to a visitor and being carried into the local community.

A list with each apartment number, resident name and the two Essential Visitors names will be kept at the front entrance to refer to when EVs are visiting.

Our amazing caregivers and management teams have worked hard to keep the COVID-19 virus out of our BAKA Communities. Unfortunately, we have several residents and two staff test positive for COVID-19 at Beaver Dam Memory Care on or before Nov. 10th, 2021. For this reason, we will be restricting visitation until 11/22/2021. Please see our visitation definition below and note that Beaver Dam Memory Care will be Closed to Visitation and under our Community Spread Precaution Visitation policy until further notice.

Community Spread Precaution Visitation: requires all visitors to comply with the following:

  • Non-medical in-person visits will NOT be allowed inside the community
  • Patio and Window Visits permitted
    • Visits must be conducted in the community’s designated visitations areas only.
    • Visits must be pre-scheduled with the community receptionist.
    • Visiting hours for patio and window visits are limited to 10:00 a.m. to 6:15 p.m. (Monday – Sunday)
      • Please limit your visit to 15 minutes
      • Limited hours are to protect residents from increased exposure
    • Wear a mask at all times while visiting
      • Remain at least 6 feet from residents at all times during the visit – no touching, hugging, kissing
    • Only two family members allowed per visit.
    • Visitors who do not comply with the visitation guidelines will be required to leave the community and not allowed to return until January 1, 2022, when communicable disease status will be reassessed – no exceptions will be made!!
  • Virtual Visits offered
    • Visits must be pre-scheduled with community receptionist. Community will supply resident with device needed to complete this type of visit.
    • Daily Visitation Hours
      • 9:00 a.m. - 11:00 a.m., and 1:30 p.m. - 4:00 p.m.
    • Zoom and Skype are the only two virtual platforms offered by the community at this time
    • Please limit your visit to 15 minutes
  • Hospice/Home Health/Visiting Physicians: Visits will be conducted, Provider must don full PPE (agency provides) at the entrance of the community, be strictly screened for symptoms/temp, etc. May not use public restrooms or resident bathrooms other than to wash hands and use single use toweling.
  • Family Visitation for Resident on Hospice: If a resident is on hospice and is actively passing away, community will permit two designated family members to visit the resident inside of the resident’s apartment. The two designated family members will be expected too successfully pass a COVID-19 screening before each visit and wear a mask in the community at all times. The two designated family members may not relinquish their visitation privileges to any other family members at any time.
  • Therapy: Allowed in community if agency only serves specific community. Outside therapists who serve other communities must don full PPE (agency provides) at the entrance of the community, be strictly screened for symptoms/temp, etc. May not use public restroom or resident bathrooms other than to wash hands and use single use toweling.

 

  • Residents may attend medically necessary appointments only
    • If community is not transporting resident to appointment, individual transporting resident will be screen at Reception prior to resident leaving for appointment
      • Those with symptoms or a temperature over 100 degrees Fahrenheit will not be allowed to transport resident
    • Both resident and transporting individual must wear mask entire time outside of community
    • Resident will be screened upon return at Reception prior to proceeding into the community
  • On-site beautician: Allowed in community if beautician(s) only serves specific community. Beautician(s) must don full PPE (beautician(s) supplies) at the entrance of the community, be strictly screened for symptoms/temp, etc. May not use public restrooms or resident bathrooms other than to wash hands and use single use toweling.

Resident safety is our first priority, and we are doing everything we can to keep COVID-19 out of the communities. We are staying in close communication with local and state health officials and are actively disinfecting all surfaces several times per day. Staff are screened at the start of each shift and those with a temperature equal to or greater than 100 degrees Fahrenheit, or with 2 or more symptoms, are not allowed into the community. Staff are paid while sick or waiting for test results. This encourages our staff to tell us if they are unwell and to stay home.

If there is an active or suspected case in a community, we will screen residents daily for symptoms, stop all activities and communal dining (in non-dementia units) and we will ask residents to remain in their rooms at all times. Meals will be delivered to resident rooms at no charge.

We understand you are concerned about your loved one, but we must restrict visitation to reduce the spread of the virus. Staying connected with your loved one is important, so we encourage you to call, text and video chat. Our dedicated staff can assist with this.

Dear Residents, Family and Friends of BAKA Communities:

Thank you for your patience over the past few weeks, as our teams worked with local pharmacies to administer both doses of the Moderna COVID-19 vaccine.  We are happy to announce that we have successfully completed both doses of the vaccine in all of our communities - allowing us to move forward with initiating our new Essential Visitor policy.  This new policy will roll out in each community 21 days after the second dose was administered.  Please refer to the chart and EV visitation guidelines that follow to see when your community will be open to this new form of visitation.

In the meantime, if you need to visit your loved one before the EV policy starts, we will permit one (1) designated visitor to enter the community. The designated visitor will only be permitted to visit one day per week, for one hour maximum.  **Visiting Hours will be Monday through Friday, 9am – 12p & 1pm-5pm**


The temporarily designated visitor will be required to
:

  • Enter the community wearing a facemask, or face shield. Face covering must be worn at ALL times (including in the resident apartment).
  • Be screened for symptoms, which will include: Answering a symptom questionnaire, taking your temperature, and successfully completing a nasal COVID-19 rapid test. Entry to the community will NOT be permitted if any of the screening protocols are not successfully completed.
  • Pick up a laminated visitor’s pass at the front entrance and always wear it in a visible place at all times AND return it to the container marked “badges to be sanitized” prior to leaving the community (located at the entrance/exit). Badges taken out of the community will not be replaced and must be returned to the community to continue visiting during periods of quarantine.

Dear Residents, Family and Friends of BAKA Communities:

As we continue to anticipate the second dose of the COVID-19 vaccine our visitation will remain limited to essential health care providers, unless you absolutely NEED to visit with your loved one(s) in-person before the second dose of the vaccine have been administered. As of today, all of the BAKA communities will have received their first dose of the COVID-19 vaccine. We are excited that we have completed the first dose and are looking forward to the second one being administered in the coming weeks. Our visitation remains limited to essential health care providers unless you absolutely NEED to visit with your loved one(s) in-person before the second dose of the vaccine have been administered. Dear Residents, Family and Friends of BAKA Communities:

As you may know, earlier this week we had to postpone our much anticipated “Essential Visitor” (EV) visitation policy until further notice.  This was a difficult decision for us to make and is based on the combination of recent survey results, as well as new information surrounding the availability of the COVID-19 vaccines in our communities.  The survey results revealed most families/residents are not comfortable opening to EV’s until the vaccines have been made available to all our residents.  Coincidentally, over the weekend it was brought to our attention that our communities would be eligible to receive the vaccines sooner than originally anticipated.  For these two reasons combined we made the difficult decision to postpone the EV visitation at this time.  We will revisit our visitation policies after the initial vaccines have been administered in all BAKA communities.

We do apologize for the communication delay and any inconvenience it may have caused for you and your family.  Our decision to postpone this visitation policy was not taken lightly, as we understand that you would all enjoy the opportunity to visit with your loved one(s) in person.  Although we cannot fully open to EV’s at this time, we realize the past nine months have taken an emotional toll on everyone, especially our residents. So, starting January 15th, if you truly NEED to visit with your loved one(s) in-person before the vaccines have been administered, we will permit one (1) designated visitor to enter the community. Dear Residents, Family and Friends of BAKA Communities:

NEW UPDATE:

The COVID-19 virus is very serious but so is our resident’s need to see loved ones.  The depression and isolation associated with no communal meals or activities is mounting in our community and across the country.  With this in mind, we crafted a policy and procedure that will begin next Monday, January 11, 2021.  Each resident will be asked to designate two “Essential Visitors” who will be allowed into the community even during quarantined periods.  The Essential Visitors will not be allowed into an apartment when the resident is positive for COVID.  This is to keep the virus from spreading to a visitor and being carried into the local community.

A list with each apartment number, resident name and the two Essential Visitors names will be kept at the front entrance to refer to when EVs are visiting.

Essential Visitors (EVs):

  • May visit any day of the week between 8 a.m. and 8 p.m. (this includes weekends and holidays).
    • There may be extended wait times for a staff member to answer the door and provide screening/badge services if visitors come when there is no receptionist. Please check dates and times receptionists are assigned to front entrances to avoid waiting.
  • Must be screened for symptoms prior to entering the community for each visit
  • Must pick up a laminated visitor’s pass at the front entrance and wear it in a visible place at all times AND return it to the container marked “badges to be sanitized” prior to leaving the community (located at the entrance/exit).   • Must do everything possible to protect our residents and staff – wear a mask, wash hands, visit wearing clean clothing and outerwear, stay 6 feet apart, etc.
  • May not visit during meal times (8a-9a), (12p-1p) & (5p-6p)
  • May not relinquish this position to other family members or friends.
  • May bring in supplies, gifts, personal products, etc. for resident use.

This virus has gone on too long and we are implementing changes to ensure the well-being of our residents and our family members.  The goal is to help residents stay connected and assure families their loved ones are receiving good care and are safe.  Please join us in launching this visitation in a safe and secure manner so everyone can have someone visit every day with no restrictions.  If we all work together, we can open up enough to keep our residents and families connected and our community COVID free.

Additional changes:

  • Beauticians will be returning to the communities effective January 11, 2021 and must be swabbed and tested prior to entering.
  • Church service personnel will be swabbed and tested prior to entering the community and will provide church services to those negative for COVID. Residents attending services must wear masks at all times.  No physical communion will be offered at this time.
  • Hospice personnel and home health services are allowed in the community at all times but must be screened, masked and sanitized prior to entering and must be tested by their individual agencies prior to visiting residents in their apartments.

Our amazing caregivers and management teams have worked hard to keep the COVID-19 virus out of our BAKA Communities.  Unfortunately, rising numbers in Wisconsin have everyone scared and worried.  For this reason, we will post a weekly update every Wednesday on our Facebook Pages and the BAKA website.  We will also post the weekly update at the front entrances of our communities.  (See the last page of this update to view the visitation status of each community).

Community Spread Precaution Visitation:   requires all visitors to comply with the following:

  • Non-medical in-person visits will NOT be allowed inside the community
  • Patio and Window Visits permitted
    • Visits must be conducted in the community’s designated visitations areas only.
    • Visits must be pre-scheduled with the community receptionist.
    • Visiting hours for patio and window visits are limited to 10:00 a.m. to 6:15 p.m. (Monday – Sunday)
      • Please limit your visit to 15 minutes
      • Limited hours are to protect residents from increased exposure
    • Wear a mask at all times while visiting
      • Remain at least 6 feet from residents at all times during the visit – no touching, hugging, kissing
    • Only two family members allowed per visit.
    • Visitors who do not comply with the visitation guidelines will be required to leave the community and not allowed to return until January 1, 2021 when communicable disease status will be reassessed – no exceptions will be made!!
  • Virtual Visits offered
    • Visits must be pre-scheduled with community receptionist. Community will supply resident with device needed to complete this type of visit.
    • Daily Visitation Hours

    • Resident will be screened upon return at Reception prior to proceeding into the community

Beautician(s) must don full PPE (beautician(s) supplies) at the entrance of the community, be strictly screened for symptoms/temp, etc.  May not use public restrooms or resident bathrooms other than to wash hands and use single use toweling.

  • Come directly from home wearing freshly laundered clothing
  • Empty bladder/bowel prior to visiting – visitors MAY NOT use public bathrooms or resident bathrooms
  • Be screened at Reception prior to proceeding into the community
  • Be screened for COVID-19 symptoms including temperature
    • Those with symptoms or a temperature over 100 degrees Fahrenheit will not be allowed entry to the community
  • Wear a mask at all times while in the community, including inside resident apartments
    • Visitors who do not comply with the visitation guidelines will be required to leave the community and not allowed to return until January 1, 2021 when communicable disease status will be reassessed – no exceptions will be made!!
    • Remain at least 6 feet from residents at all times during the visit
  • Go directly from Reception to the resident’s apartment
  • May not be in the common spaces of the community, including hallways, except when going to / from resident apartment
  • Visiting hours are limited to: 10 a.m. to noon AND 2 p.m. to 4 p.m. (Monday – Sunday)
    • Limited hours are to protect staff from visitor exposure when passing medications or providing cares. Staff will not enter apartments during these hours unless medically necessary or requested by the resident.
    • No visitation is allowed on Housekeeping/Shower/Laundry day to allow staff to clean apartment, strip and re-make bed(s) and complete laundry without exposure to outside visitors.
    • Only two visitors permitted inside of resident’s apartment to ensure safe social distancing

Closed to Visitation:  bans all visitors (including therapists) except the following:

  • Hospice: Visits will be conducted via Zoom unless resident is actively passing in which case, Hospice RN must don full PPE (Hospice provides) at the entrance of the community, be screened for symptoms/temp, etc.  May not use public restrooms or resident bathrooms other than to wash hands and use single use toweling.
  • Home Health Providers: May enter for life safety (catheter change / wound care) reasons after complying with all bullet statements under “Open to Visitation” above.
  • Visiting Physicians: Must comply with all bullet statements under “Open to Visitation” above.

Resident safety is our first priority, and we are doing everything we can to keep COVID-19 out of the communities.  We are staying in close communication with local and state health officials and are actively disinfecting all surfaces several times per day.  Staff are screened at the start of each shift and those with a temperature equal to or greater than 100 degrees Fahrenheit, or with 2 or more symptoms, are not allowed into the community.  Staff are paid while sick or waiting for test results.  This encourages our staff to tell us if they are unwell and to stay home.

If there is an active or suspected case in a community, we will screen residents daily for symptoms, stop all activities and communal dining (in non-dementia units) and we will ask residents to remain in their rooms at all times.  Meals will be delivered to resident rooms at no charge.

We understand you are concerned about your loved one, but we must restrict visitation to reduce the spread of the virus. Staying connected with your loved one is important, so we encourage you to call, text and video chat.   Our dedicated staff can assist with this.

Dear Residents, Family and Friends of BAKA Communities:

Our amazing caregivers and management teams have worked hard to keep the COVID-19 virus out of our BAKA Communities.  Unfortunately, rising numbers in Wisconsin have everyone scared and worried.  For this reason, we will post a weekly update every Wednesday on our Facebook Pages and the BAKA website.  We will also post the weekly update at the front entrances of our communities.  (See the last page of this update to view the visitation status of each community).

The next update will be posted to our Facebook Pages and the BAKA website on Wednesday, December 16, 2020.

Community Spread Precaution Visitation:   requires all visitors to comply with the following:

  • Non-medical in-person visits will NOT be allowed inside the community
  • Patio and Window Visits permitted
    • Visits must be conducted in the community’s designated visitations areas only.
    • Visits must be pre-scheduled with the community receptionist.
    • Visiting hours for patio and window visits are limited to 10:00 a.m. to 6:15 p.m. (Monday – Sunday)
      • Please limit your visit to 15 minutes
      • Limited hours are to protect residents from increased exposure
    • Wear a mask at all times while visiting
      • Remain at least 6 feet from residents at all times during the visit – no touching, hugging, kissing
    • Only two family members allowed per visit.
    • Visitors who do not comply with the visitation guidelines will be required to leave the community and not allowed to return until January 1, 2021 when communicable disease status will be reassessed – no exceptions will be made!!
  • Virtual Visits offered
    • Visits must be pre-scheduled with community receptionist. Community will supply resident with device needed to complete this type of visit.
    • Daily Visitation Hours
      • 9:00 a.m. - 11:00 a.m., and 1:30 p.m. - 4:00 p.m.
    • Zoom and Skype are the only two virtual platforms offered by the community at this time
    • Please limit your visit to 15 minutes
  • Hospice/Home Health/Visiting Physicians: Visits will be conducted, Provider must don full PPE (agency provides) at the entrance of the community, be strictly screened for symptoms/temp, etc.  May not use public restrooms or resident bathrooms other than to wash hands and use single use toweling.
  • Therapy: Allowed in community if agency only serves specific community. Outside therapists who serve other communities must don full PPE (agency provides) at the entrance of the community, be strictly screened for symptoms/temp, etc. May not use public restroom or resident bathrooms other than to wash hands and use single use toweling.
  • Residents may attend medically necessary appointments only
    • If community is not transporting resident to appointment, individual transporting resident will be screen at Reception prior to resident leaving for appointment
      • Those with symptoms or a temperature over 100 degrees Fahrenheit will not be allowed to transport resident
    • Both resident and transporting individual must wear mask entire time outside of community
    • Resident will be screened upon return at Reception prior to proceeding into the community
  • On-site beautician: Allowed in community if beautician(s) only serves specific community. Beautician(s) must don full PPE (beautician(s) supplies) at the entrance of the community, be strictly screened for symptoms/temp, etc.  May not use public restrooms or resident bathrooms other than to wash hands and use single use toweling.

Open to Visitation:   requires all visitors to comply with the following: 

  • Come directly from home wearing freshly laundered clothing
  • Empty bladder/bowel prior to visiting – visitors MAY NOT use public bathrooms or resident bathrooms
  • Be screened at Reception prior to proceeding into the community
  • Be screened for COVID-19 symptoms including temperature
    • Those with symptoms or a temperature over 100 degrees Fahrenheit will not be allowed entry to the community
  • Wear a mask at all times while in the community, including inside resident apartments
    • Visitors who do not comply with the visitation guidelines will be required to leave the community and not allowed to return until January 1, 2021 when communicable disease status will be reassessed – no exceptions will be made!!
    • Remain at least 6 feet from residents at all times during the visit
  • Go directly from Reception to the resident’s apartment
  • May not be in the common spaces of the community, including hallways, except when going to / from resident apartment
  • Visiting hours are limited to: 10 a.m. to noon AND 2 p.m. to 4 p.m. (Monday – Sunday)
    • Limited hours are to protect staff from visitor exposure when passing medications or providing cares. Staff will not enter apartments during these hours unless medically necessary or requested by the resident.
    • No visitation is allowed on Housekeeping/Shower/Laundry day to allow staff to clean apartment, strip and re-make bed(s) and complete laundry without exposure to outside visitors.
    • Only two visitors permitted inside of resident’s apartment to ensure safe social distancing

Closed to Visitation:  bans all visitors (including therapists) except the following:

  • Hospice: Visits will be conducted via Zoom unless resident is actively passing in which case, Hospice RN must don full PPE (Hospice provides) at the entrance of the community, be screened for symptoms/temp, etc.  May not use public restrooms or resident bathrooms other than to wash hands and use single use toweling.
  • Home Health Providers: May enter for life safety (catheter change / wound care) reasons after complying with all bullet statements under “Open to Visitation” above.
  • Visiting Physicians: Must comply with all bullet statements under “Open to Visitation” above.

Resident safety is our first priority, and we are doing everything we can to keep COVID-19 out of the communities.  We are staying in close communication with local and state health officials and are actively disinfecting all surfaces several times per day.  Staff are screened at the start of each shift and those with a temperature equal to or greater than 100 degrees Fahrenheit, or with 2 or more symptoms, are not allowed into the community.  Staff are paid while sick or waiting for test results.  This encourages our staff to tell us if they are unwell and to stay home.

If there is an active or suspected case in a community, we will screen residents daily for symptoms, stop all activities and communal dining (in non-dementia units) and we will ask residents to remain in their rooms at all times.  Meals will be delivered to resident rooms at no charge.

We understand you are concerned about your loved one, but we must restrict visitation to reduce the spread of the virus. Staying connected with your loved one is important, so we encourage you to call, text and video chat.   Our dedicated staff can assist with this.

Some of you may have questions regarding COVID-19.  Please email the Community Director with questions and they will respond to you as soon as possible.  We ask that you not leave phone messages.

Community Spread Precaution Visitation:   requires all visitors to comply with the following:

  • Non-medical in-person visits will NOT be allowed inside the community
  • Patio and Window Visits permitted
    • Visits must be conducted in the community’s designated visitations areas only.
    • Visits must be pre-scheduled with the community receptionist.
    • Visiting hours for patio and window visits are limited to 10:00 a.m. to 6:15 p.m. (Monday – Sunday)
      • Please limit your visit to 15 minutes
      • Limited hours are to protect residents from increased exposure
    • Wear a mask at all times while visiting
      • Remain at least 6 feet from residents at all times during the visit – no touching, hugging, kissing
    • Only two family members allowed per visit.
    • Visitors who do not comply with the visitation guidelines will be required to leave the community and not allowed to return until January 1, 2021 when communicable disease status will be reassessed – no exceptions will be made!!
  • Virtual Visits offered
    • Visits must be pre-scheduled with community receptionist. Community will supply resident with device needed to complete this type of visit.

    • Both resident and transporting individual must wear mask entire time outside of community
    • Resident will be screened upon return at Reception prior to proceeding into the community

  • Come directly from home wearing freshly laundered clothing
  • Empty bladder/bowel prior to visiting – visitors MAY NOT use public bathrooms or resident bathrooms
  • Be screened at Reception prior to proceeding into the community
  • Be screened for COVID-19 symptoms including temperature
    • Those with symptoms or a temperature over 100 degrees Fahrenheit will not be allowed entry to the community
  • Wear a mask at all times while in the community, including inside resident apartments
    • Visitors who do not comply with the visitation guidelines will be required to leave the community and not allowed to return until January 1, 2021 when communicable disease status will be reassessed – no exceptions will be made!!
    • Remain at least 6 feet from residents at all times during the visit
  • Go directly from Reception to the resident’s apartment
  • May not be in the common spaces of the community, including hallways, except when going to / from resident apartment
  • Visiting hours are limited to: 10 a.m. to noon AND 2 p.m. to 4 p.m. (Monday – Sunday)
    • Limited hours are to protect staff from visitor exposure when passing medications or providing cares. Staff will not enter apartments during these hours unless medically necessary or requested by the resident.
    • No visitation is allowed on Housekeeping/Shower/Laundry day to allow staff to clean apartment, strip and re-make bed(s) and complete laundry without exposure to outside visitors.
    • Only two visitors permitted inside of resident’s apartment to ensure safe social distancing

Closed to Visitation:  bans all visitors (including therapists) except the following:

  • Hospice: Visits will be conducted via Zoom unless resident is actively passing in which case, Hospice RN must don full PPE (Hospice provides) at the entrance of the community, be screened for symptoms/temp, etc.  May not use public restrooms or resident bathrooms other than to wash hands and use single use toweling.
  • Home Health Providers: May enter for life safety (catheter change / wound care) reasons after complying with all bullet statements under “Open to Visitation” above.
  • Visiting Physicians: Must comply with all bullet statements under “Open to Visitation” above.

Resident safety is our first priority, and we are doing everything we can to keep COVID-19 out of the communities.  We are staying in close communication with local and state health officials and are actively disinfecting all surfaces several times per day.  Staff are screened at the start of each shift and those with a temperature equal to or greater than 100 degrees Fahrenheit, or with 2 or more symptoms, are not allowed into the community.  Staff are paid while sick or waiting for test results.  This encourages our staff to tell us if they are unwell and to stay home.

If there is an active or suspected case in a community, we will screen residents daily for symptoms, stop all activities and communal dining (in non-dementia units) and we will ask residents to remain in their rooms at all times.  Meals will be delivered to resident rooms at no charge.

We understand you are concerned about your loved one, but we must restrict visitation to reduce the spread of the virus. Staying connected with your loved one is important, so we encourage you to call, text and video chat.   Our dedicated staff can assist with this.

Resident safety is our first priority, and we are doing everything we can to keep COVID-19 out of the communities.  We are staying in close communication with local and state health officials and are actively disinfecting all surfaces several times per day.  Staff are screened at the start of each shift and those with a temperature equal to or greater than 100 degrees Fahrenheit, or with 2 or more symptoms, are not allowed into the community.  Staff are paid while sick or waiting for test results.  This encourages our staff to tell us if they are unwell and to stay home.

If there is an active or suspected case in a community, we will screen residents daily for symptoms, stop all activities and communal dining (in non-dementia units) and we will ask residents to remain in their rooms at all times.  Meals will be delivered to resident rooms at no charge.

We understand you are concerned about your loved one, but we must restrict visitation to reduce the spread of the virus. Staying connected with your loved one is important, so we encourage you to call, text and video chat.   Our dedicated staff can assist with this.

Our amazing caregivers and management teams have worked hard to keep the COVID-19 virus out of our BAKA Communities.  Unfortunately, rising numbers in Wisconsin have everyone scared and worried.  For this reason, we will post a weekly update every Wednesday on our Facebook Pages and the BAKA website.  We will also post the weekly update at the front entrances of our communities.  (See the last page of this update to view the visitation status of each community).

Community Spread Precaution Visitation:   requires all visitors to comply with the following:

  • Non-medical in-person visits will NOT be allowed inside the community
  • Patio and Window Visits permitted
    • Visits must be conducted in the community’s designated visitations areas only.
    • Visits must be pre-scheduled with the community receptionist.
    • Visiting hours for patio and window visits are limited to 10:00 a.m. to 6:15 p.m. (Monday – Sunday)
      • Please limit your visit to 15 minutes
      • Limited hours are to protect residents from increased exposure
    • Wear a mask at all times while visiting
      • Remain at least 6 feet from residents at all times during the visit – no touching, hugging, kissing
    • Only two family members allowed per visit.

Our amazing caregivers and management teams have worked hard to keep the COVID-19 virus out of our BAKA Communities. Unfortunately, rising numbers in Wisconsin have everyone scared and worried. For this reason, we will post a weekly update every Wednesday on our Facebook Pages and the BAKA website. We will also post the weekly update at the front entrances of our communities. Open to Visitation: requires all visitors to comply with the following:

  • Come directly from home wearing freshly laundered clothing
  • Empty bladder/bowel prior to visiting – visitors MAY NOT use public bathrooms or resident bathrooms
  • Be screened at Reception prior to proceeding into the community
  • Be screened for COVID-19 symptoms including temperature

For this reason, we will post a weekly update every Wednesday on our Facebook Pages and the BAKA website. We will also post the weekly update at the front entrances of our communities. (See the last page of this update to view the visitation status of each community).

Open to Visitation: requires all visitors to comply with the following:

  • Come directly from home wearing freshly laundered clothing
  • Empty bladder/bowel prior to visiting – visitors MAY NOT use public bathrooms or resident bathrooms
  • Be screened at Reception prior to proceeding into the community
  • Be screened for COVID-19 symptoms including temperature
    • Those with symptoms or a temperature over 100 degrees Fahrenheit will not be allowed entry to the community
  • Have bottoms of shoes disinfected (each shoe sprayed for 5 seconds)
  • Wear a mask at all times while in the community, including inside resident apartments
    • Visitors caught inside the community not wearing a mask (including in resident apartments) will be banned from visiting the community until January 1, 2021 when communicable disease status will be reassessed – no exceptions will be made!!
    • Remain at least 6 feet from residents at all times during the visit
  • Go directly from Reception to the resident’s apartment
  • May not be in the common spaces of the community, including hallways, except when going to / from resident apartment
  • Visiting hours are limited to: 10 a.m. to noon AND 2 p.m. to 4 p.m. (Monday – Sunday)
    • Limited hours are to protect staff from visitor exposure when passing medications or providing cares. Staff will not enter apartments during these hours unless medically necessary or requested by the resident.
    • No visitation is allowed on Housekeeping/Shower/Laundry day to allow staff to clean apartment, strip and re-make bed(s) and complete laundry without exposure to outside visitors.
    • Only two visitors permitted inside of resident’s apartment to ensure safe social distancing

Closed to Visitation: bans all visitors (including therapists) except the following:

  • Hospice: Visits will be conducted via Zoom unless resident is actively passing in which case, Hospice RN must don full PPE (Hospice provides) at the entrance of the community, be screened for symptoms/temp, etc. May not use public restrooms or resident bathrooms other than to wash hands and use single use toweling.
  • Home Health Providers: May enter for life safety (catheter change / wound care) reasons after complying with all bullet statements under “Open to Visitation” above.
  • Visiting Physicians: Must comply with all bullet statements under “Open to Visitation” above.

Resident safety is our first priority and we are doing everything we can to keep COVID-19 out of the communities. We are staying in close communication with local and state health officials and are actively disinfecting all surfaces several times per day. Staff are screened at the start of each shift and those with a temperature equal to or greater than 100 degrees Fahrenheit, or with 2 or more symptoms, are not allowed into the community. Staff are paid while sick or waiting for test results. This encourages our staff to tell us if they are unwell and to stay home.

If there is an active or suspected case in a community, we will screen residents daily for symptoms, stop all activities and communal dining (in non-dementia units) and we will ask residents to remain in their rooms at all times. Meals will be delivered to resident rooms at no charge.

We understand you are concerned about your loved one, but we must restrict visitation to reduce the spread of the virus. Staying connected with your loved one is important, so we encourage you to call, text and video chat. Our dedicated staff can assist with this.

Dear Residents, Family and Friends of BAKA Communities:

Our amazing caregivers and management teams have worked hard to keep the COVID-19 virus out of our BAKA Communities.  Unfortunately, rising numbers in Wisconsin have everyone scared and worried.  For this reason, we will post a weekly update every Wednesday on our Facebook Pages and the BAKA website.  We will also post the weekly update at the front entrances of our communities.  (See the last page of this update to view the visitation status of each community).

The next update will be posted to our Facebook Pages and the BAKA website on Wednesday, August, 5, 2020.

Open to Visitation:   requires all visitors to comply with the following: 

  • Come directly from home wearing freshly laundered clothing
  • Empty bladder/bowel prior to visiting – visitors MAY NOT use public bathrooms or resident bathrooms
  • Be screened at Reception prior to proceeding into the community
  • Be screened for COVID-19 symptoms including temperature
    • Those with symptoms or a temperature over 100 degrees Fahrenheit will not be allowed entry to the community
  • Have bottoms of shoes disinfected (each shoe sprayed for 5 seconds)
  • Wear a mask at all times while in the community, including inside resident apartments
    • Visitors caught inside the community not wearing a mask (including in resident apartments) will be banned from visiting the community until January 1, 2021 when communicable disease status will be reassessed – no exceptions will be made!!
    • Remain at lease 6 feet from residents at all times during the visit
  • Go directly from Reception to the resident’s apartment
  • May not be in the common spaces of the community, including hallways, except when going to / from resident apartment
  • Visiting hours are limited to: 10 a.m. to noon AND 2 p.m. to 4 p.m. (Monday – Sunday)
    • Limited hours are to protect staff from visitor exposure when passing medications or providing cares. Staff will not enter apartments during these hours unless medically necessary or requested by the resident.
    • No visitation is allowed on Housekeeping/Shower/Laundry day to allow staff to clean apartment, strip and re-make bed(s) and complete laundry without exposure to outside visitors.

Closed to Visitation:  bans all visitors (including therapists) except the following:

  • Hospice: Visits will be conducted via Zoom unless resident is actively passing in which case, Hospice RN must don full PPE (Hospice provides) at the entrance of the community, be screened for symptoms/temp, etc.  May not use public restrooms or resident bathrooms other than to wash hands and use single use toweling.
  • Home Health Providers: May enter for life safety (catheter change / wound care) reasons after complying with all bullet statements under “Open to Visitation” above.
  • Visiting Physicians: Must comply with all bullet statements under “Open to Visitation” above.

Resident safety is our first priority and we are doing everything we can to keep COVID-19 out of the communities.  We are staying in close communication with local and state health officials and are actively disinfecting all surfaces several times per day.  Staff are screened at the start of each shift and those with a temperature equal to or greater than 100 degrees Fahrenheit, or with 2 or more symptoms, are not allowed into the community.  Staff are paid while sick or waiting for test results.  This encourages our staff to tell us if they are unwell and to stay home.

If there is an active or suspected case in a community, we will screen residents daily for symptoms, stop all activities and communal dining (in non-dementia units) and we will ask residents to remain in their rooms at all times.  Meals will be delivered to resident rooms at no charge.

We understand you are concerned about your loved one, but we must restrict visitation to reduce the spread of the virus. Staying connected with your loved one is important, so we encourage you to call, text and video chat.   Our dedicated staff can assist with this.

3/19/2020:

If you wish to drop off supplies for your loved one, please do the following:

Bag your items in a plastic bag, tied at the top. Label the bag clearly using masking tap or by stapling a piece of paper with your loved ones full name and apartment number.

Bring your bag(s), one at a time to the exterior entrance door of the building and place it down.  Return to your car. Staff will be out to gather the supplies and take them to your loved one.

Supply drop off hours are:

  • Monday thru Friday: 2 pm to 4 pm
  • Saturday/Sunday: 3 pm to 4 pm

Please be certain not to come within 6 ft of other individuals that are also dropping off supplies.

You may also direct ship items to the building.

3/19/2020:

As you are aware, Personal Protective Equipment is extremely difficult to find. Most supplies are being alotted to hospitals (where they need to be); however, as healthcare providers with a high risk population, we are finding ourselves unable to locate PPE through our normal vendors and other providers. The Strategic National Supply has been opened, but unfortunately, assisted livings will receive minimal PPE from this release.

Therefore, we are seeking donations of the following items:

  • N95 respirators or other masks used in construction or by painters, automotive personnel, etc.
  • Isolation Masks (Surgical Masks)
  • Isolation Gowns ( as well as disposable painters coveralls with hoods or reusable rainsuits with hoods)
  • Shoe Covers (these can be painters, plumbers, etc. shoe covers)

We would like to thank all those in the community for your understanding and assistance during the unprecedented time.

As the Coronavirus (COVID-19) continues to spread throughout the world and begins to impact our surrounding communities, we want to assure you that BAKA Enterprises is taking all the necessary precautions to ensure the safety and well being of the residents and staff in all of our communities.   Even though COVID-19 can infect anyone of any age, it is especially dangerous for those with living with a compromised immune system, as well as our elderly population.  For these reasons we are taking special precautions with all visitors and residents in the upcoming weeks.

We are committed to protecting the health of our residents and staff, which is why our team has been working tirelessly to review, discuss and implement suggestions from the Centers for Disease Control and Prevention (CDC), The World Health Organization (WHO), Wisconsin Department of Health Services (WisDHS), and The Centers for Medicare and Medicaid Services (CMS).

As we continue to protect our communities from potential exposure to COVID-19, we are implementing the following  - effective immediately:

    • Restrict community entrance to only:
      • Medically necessary personnel
      • Facility staff
      • Those entering to assist with end of life care
      • Implement a Facility Staff / Visitor Questionnaire
    • Restrict resident non-medically necessary outings until further notice:
      • No resident outings, unless medically necessary
      • If resident leaves for a non-medically necessary outing, re-entry to the community may be refused.
      • Review and print “Resident Acknowledgement of No Outings” for additional details.
    • Pandemic Plan
      • Plan consists of four phases
      • Developed based on recommendations from the CDC, WHO, and WisDHS
      • Review and print “Pandemic Plan” for additional information
      • All BAKA communities are currently in Phase 1: Precautionary Measures (No active cases)
    • Volunteer Recruitment
      • We must be prepared for potential staff shortages should COVID-19 reach the Pandemic Phase in a community
      • Seeking Lead Volunteer to coordinate all volunteers
      • Seeking volunteers ages 16 or older to assist in daily, non-medical community tasks should we experience a staff shortage due to COVID-19
      • Review and print “Volunteer Recruitment Form” for additional information.

The World Health Organization has declared a global pandemic concerning the Coronavirus. The Center for Disease Control has advised us to limit non-essential personnel visits to our Assisted Living and Memory Care communities at this time. Please DO NOT enter a community if you are not an essential personnel team member.

Helpful ways to protect you and your family

Stay safe and prepared with these simple tips

      • Wash your hands with soap often and for at least 20 seconds
      • Avoid heavily public areas and touching eyes, nose, and mouth
      • Stay at home even if you develop mild symptoms like a headache or slightly runny nose until you recover
      • If you develop a fever, cough, or difficulty breathing, call ahead to your local MD or Clinic, make them aware of your symptoms and request guidance.

Dear Residents and Family Members:

The Coronavirus (COV-19 Virus) is now active in the U.S., including cases in Wisconsin.  Coronavirus is similar to other communicable diseases such as Influenza and Norovirus in the way it is spread and in the way we can slow or prevent the spread.

To protect the residents and staff in our community, we are requiring all visitors and employees adhere to the following precautionary measures:

      1. COVER your nose and mouth with a tissue when coughing or sneezing. Immediately discard the tissue in a trash receptacle.
      2. WASH your hands with soap and water for a minimum of twenty seconds. Especially after you cough or sneeze.
        1. CDC also recommends frequent use of hand sanitizer (keep it in your vehicle, at work, home).
      3. AVOID contact with ill people.
      4. STAY HOME if you feel sick or are experiencing symptoms provided on the enclosed checklist. This reduces the spread of the virus to others.
      5. DO NOT TOUCH your eyes, nose, mouth or face unless you wash your hands immediately after.
      6. NOTIFY our community immediately if you have visited within 14 days and develop symptoms (see checklist) or are diagnosed with Coronavirus.
      7. DO NOT VISIT the community if you feel ill, have a fever, are coughing, sneezing, short of breath, feel extreme fatigue, or have experienced diarrhea and/or vomiting.

In case you become ill, the Department of Health Services advises everyone to prepare for home isolation by purchasing enough supplies for 14 days including food, water, comfort medications, etc.

What happens if there is a confirmed Coronavirus case in the Assisted Living community?

    1. Symptomatic residents must wear a mask and remain in their apartment or specified isolation area until symptoms have ended.
    2. The Public Health Department will be notified immediately.
    3. Residents and Families will be notified as soon as possible via the community Facebook page and posters at the front entrance.
    4. Access to the community will be restricted to staff and health care workers.
    5. All trips to non-essential medical appointments, shopping and events will be canceled until the community has been cleared by the public health department.
    6. Residents will communicate with friends and family electronically (Skype, Duo, Whats App, etc.)
    7. Family members will be required to communicate with the community via email to keep phone lines open for Health Department workers, 911 and other needs.
